This service will require that the patient’s Primary Care Physician (PCP), Psychiatrist, or Mental Health Nurse Practitioner sign off on the report from Free Life Counseling Services.

This diagnostic evaluation is NOT a neuropsychological evaluation, which New York State requires for Social Security Income/Disability Income.
